Directions

We are located at the northeast corner of Highway 96 and Lexington Avenue. We are directly behind the US Bank in a large brick building noted as the Shoreview Medical Center.

• From the North: Take 35 W South to Highway 96 Exit. Turn East and follow through until Lexington Avenue intersection. Take your first Left after intersection at Churchill Street. 
  
  
• From the South: Take 35 E North to 694 West. Take the Lexington Avenue Exit and head North approximately 1 Mile to the intersection Highway 96 and Lexington Avenue. Turn East for 2/10 mile and take your first Left onto Churchill Street.
  
  
• From the East: Take 694 West to Lexington Avenue Exit. Head North approximately 1 Mile to the intersection Highway 96 and Lexington Avenue. Turn East for 2/10 mile and take your first Left onto Churchill Street.
  
  
• From the West: Take 694 East to Lexington Avenue Exit. Head North approximately 1 Mile to the intersection Highway 96 and Lexington Avenue. Turn East for 2/10 mile and take your first Left onto Churchill Street.

Parking
Parking for patients is located on the North side of the building, off of Tomlyn Avenue. 
Parking is located for employees on the South side of the building. (Facing Highway 96.)
